Support for SermonSpeaker 4.x is canceled as it is for Joomla 2.5. I will gladly assist you with minor issues but I will not fix any bugs anymore in this releases.
The fix is to upgrade to SermonSpeaker 5.x and Joomla 3.x, which is better anyway.

I can tell you how SermonSpeaker will try to find the file.
- Take the path from the database
- Make sure there is a leading slash ('/') to this path
- Add the Joomla root directory (as stored in the constant 'JPATH_ROOT') in front of the path
- Check if the file exists with the Joomla function JFile::exists()

hmmmm, that's an old url. Is one url cached/saved for downloads and another generated dynamically for flash?

Nope, nothing is cached. Everything is taken directly from the database.

The problem was with the audio file url. I started off with Joomla in a sub folder (so all urls were prefixed with ../) and was only using Joomla for sermon speaker. I then moved Joomla to the root and used it across our site so the ../ was no longer needed. I removed the leading ../ and downloads are working now.
I'm still puzzled as to why the flash player worked...
