Support for SermonSpeaker 4.x is canceled as it is for Joomla 2.5. I will gladly assist you with minor issues but I will not fix any bugs anymore in this releases.
The fix is to upgrade to SermonSpeaker 5.x and Joomla 3.x, which is better anyway.

It's not possible with the available layouts, but it sure would be possible if you do your own layout (and know a bit of PHP and Joomla).

Difficulty is a relative term
The biggest problem is that there is no view which already holds all the data you need (speakers grouped by category).
I would probably start from the categories view and then load the speakers for each category using the appropriate model in the layout. Kind of like it's done in the series/sermons view.
