Support for SermonSpeaker 4.x is canceled as it is for Joomla 2.5. I will gladly assist you with minor issues but I will not fix any bugs anymore in this releases.
The fix is to upgrade to SermonSpeaker 5.x and Joomla 3.x, which is better anyway.

It looks as their documentation is still as crappy as it was when I first looked...

By copying your example I got it so far that it does copy the tables, but I doesn't show up as an installed extension and I don't see where I can define what package it should install. I remember why I gave up writing a plugin for it.
